Thanks for reaching out to Jotform Support. You can easily create application forms at Jotform. We have a number of useful tools and Templates Customized for Deliveries. We also have some Getting Started with Jotform guides to help you. Let me show you how you can create one by using a template:

1. On the Templates page, find a suitable template for your needs and click on the Use Template button under it.
2. Once you click it, you'll be directed to Form Builder, where you can customize your survey as want.
