Jotform Form Builder lets you create and publish forms directly on your Shopify Store. After installing and connecting the app, you can create a form, customize its fields and appearance, choose how and where it appears on your store, and publish it for your customers to access.

Installing Jotform Form Builder
First, install the Jotform Form Builder app on your Shopify store and connect your Jotform account. Here’s how:
- Log in to your Shopify Store account, and then go to the Jotform – Form Builder and click on Install on the left side of the page.
- Then, in the new tab that opens, review the requested permissions in the Install App window, and then click on Install.
- Next, click on Connect.
- Now, in the window that opens, review the requested permissions and click on Allow.
That’s it! You’ve successfully installed Jotform Form Builder on your Shopify store and are ready to start creating forms.
Creating Your First Form
Once you’ve installed and connected the app, you can create your first form from the My Forms page. This is how it’s done:
- In Jotform-Form Builder, click on My Forms in the menu on the left side of the page.
- Then, at the center of the page, click on Create Form.
- Now, in the Describe the Form You Need section, enter the description of the form you want to create, and then click on Create Form.
- To use a pre-built form, in the Templates section, select a template, and then click on Use Template.
Once your form is created, you’ll be taken to the Publish Form page, where you can choose how to display your form.
Publishing Your Form
Once your form is ready, you can configure how and where you want it to appear on your Shopify store. Here’s how to get going:
- In the menu on the left of your Publish Form page, configure these things:
- Form Display — Choose how and where the form is displayed in your store:
- Inline Section — Embed the form directly into a page or section of your store.
- Popup Modal — Display the form in a popup when customers click a button on your store.
- Floating Button — Let customers open the form using a floating button displayed on your store.
- Location Type — Select the type of store page where you want to display the form.
- Form Display — Choose how and where the form is displayed in your store:
- Then, scroll down and continue configuring these settings:
- Thank You Message — Customize what customers see after submitting your form.
- Show Thank You message — Display a thank-you message after customers submit the form.
- Redirect to a Page — Redirect customers to another page after they submit the form.
- Advanced Settings — Click on it to open the Form Design Studio, where you can customize the appearance of your form using the available design options.
- Thank You Message — Customize what customers see after submitting your form.
- Next, scroll up and then click on Publish at the top-right side of the page.
- Now, in the Shopify Theme Editor, review the form placement and in the top-right corner of the page, click on Save.
Editing Your Form
If you want to make changes to an existing form, you can open it from the My Forms page and edit its fields and settings. Here’s what to do:
- In the menu on the left side of the Jotform – Form Builder page, click on My Forms.
- Now, hover your mouse over the form you want to edit and then click on Edit Form.
That’s it! You’ll be redirected to Form Builder, where you can continue customizing your form.
Note
Any changes you make to your form will automatically be reflected in the form added to your Shopify store.
Managing Your Forms
After creating your form, you can manage them from the My Forms page. You can rename, preview, duplicate, or delete your forms, as well as access other form options.
- In the menu on the left side of the Jotform – Form Builder page, click on My Forms.
- Now, hover your mouse over the form you want to manage, and click on Three Dots icon.
- Then, in the menu that opens, select the action you want to perform:
- Rename — Change the name of your form.
- Preview — View how your form appears before sharing it.
- View Submissions — View the responses submitted through your form. You can also view your submissions by clicking on Submissions in the menu on the left.
- Analytics — View data and insights about your form submissions.
- Duplicate — Create a copy of your form.
- Delete — Move the form to Trash.

We would like to create on form for a official competition and with that create visuable statistics for all followers. - Is it possible to make that work with the forms?
If the users are adding data like 30 or 33 eor 41 etc is it possible to create a instant feedback in the ratings for the users?

Is there a way to connect your jotform to shopify so that when they hit the submit button it will create an order in my admin panel? I currently have it going to PayPal, and it is very inconvenient.

1. My customer logs onto my website with a passcode (this are wholesale customers).
2. They go to a page with a form on it that has products, variants, prices listed and they check off or type in what they want.
3. The order is totaled at the bottm and then click "submit"
4. Order is processed via Shopify/Stripes with their credit card.
5. We are notified via email from our website that an order was placed and a link to their completed form is shown on the screen.
or
1-3 is the same
4. Their form is emailed to us and we check inventory, send an email to the customer telling them that we received their order and ask them to contact us with credit card info.
5. They call us and we process the order via our website Shopify/Stripes with their given credit card #.
Possible??
